Establishment of a direct communication between the stomach and the jejunum- this anastomosis may be stapled or hand-sewn
What is the biliopancreatic diversion with duodenal switch (BPD/DS)?
BPD/DS is a combination restrictive and malabsorptive procedure that reduces the size of the stomach by 70%. The pouch is still a bit larger than with other procedures that use restriction. By connecting the pouch to the lower part of the small intestine, absorption of calories and nutrients is greatly reduced.
Current thinking is that revisional operations do not work very well and for that reason they are reserved for unique cases. As with many things in surgery, much depends on the circumstances and the available options, some of which we’ll describe below. In any case, I strongly suggest that patients return to their original surgeon who performed their first operation, as every surgeon individualizes the digestive arrangements in a particular manner. The only revisional operation I regularly perform is for patients who have a failed laparoscopic gastric banding. Those patients may benefit from having the band removed and then converting to a gastric bypass or a gastric sleeve.
The small intestine is a long tube that is approximately 18 feet long (6 meters). The small intestine is called small because the diameter or the width of the tube is much less than the large intestine. The parts of the small intestine include the duodenum, jejunum and the ileum. The duodenum is a segment of intestine between the stomach and the jejunum that is very active in digestion where many different enzymes mix from the stomach, liver, gallbladder and pancreas.
What occurs during laparoscopic gastric bypass?
Roux-en-Y Gastric Bypass works by temporarily decreasing hunger and cravings, restricting the amount of food you can eat at one time and reducing the absorption of calories. The Roux-en-Y Gastric Bypass is considered by many experts to be the current gold standard procedure for bariatric surgery. Although the surgery is typically not reversible, it is the most popular form of bariatric surgery.
At your second or pre-surgical office visit with Dr. Belsley, you will take a short quiz so that you can have an additional discussion regarding various important issues prior to your operation. The purpose of the quiz is to assist your understanding and provide a source of conversation regarding the risks and benefits of the procedure. You will also be asked to review, initial and sign a consent form that discusses a number of important possibilities of your operation.
